This is why we conduct immigration physical examinations as well as provide access to basic healthcare services.
Services We Provide
At CQC our physicians serve as Civil Surgeons that provide the highest quality immigration physical examinations to assure the completion of your paperwork for permanent residency and or other types of visas. Your health and safety is our priority! We work with the U.S. Center for Disease Control and Prevention (CDC) to reduce your risks of contagious infectious diseases and other debilitating physical or mental health conditions.
Our physicians will conduct extensive physical and mental health examinations and help you organize and collect all necessary vaccinations. If you have not received a particular vaccine, our staff will help guide you in getting it complete. Once approved, your I-693 application form will be completed by our staff and provide to you in sealed envelopes. If you are seeking an immigration physical exam please schedule an appointment today!

Immigration Medical Exam (USCIS – 1693) Procedure
You will need to present:
1. Valid Identification (Passport / ID Card)
2. Vaccination records
Payment
The upfront cost for the immigration physical and completion of all immigration paperwork is
$350 (nonrefundable)
This amount must be paid before any assessment of your vaccination documents or records.
This amount must be paid by cash, major credit/debit card or money order
Process
- The timeframe for the immigration process and completion of all paperwork varies depending on how quickly you complete all requirements.
- After registration and submission of all your vaccination records, you may be required to set up an appointment for additional vaccines and required blood tests.
- Vaccinations, if required, are scheduled based on availability. Vaccinations are not included in the fee paid above.
- Required blood tests are based on age and include tuberculosis and STD screening. Blood tests are not included in the fee paid above.
- The second visit is for the physical exam with the physician.
- Final arrangements to pick up completed forms are made after your visit (Expect a 24-48-hour turnaround time once all required vaccinations and/or blood tests are completed).
Immigration Checklist
- Pay $350 to CQC for Immigration USCIS medical exam and processing fee
- Submit ID and current vaccination records for review
- Fill out USCIS forms and necessary paperwork
- Complete vaccines/lab test requirements (charged separately)
- Promptly return results/records of test completed to Coupet Quality Clinic
- Schedule your in-office physical examination
- Final visit to pick up paperwork and pay any final fees if subjected
