Home Visits are Our Services Brought to You!
If you can not come to us, we will come to you. We offer home visits for our Direct Primary Care members that is tailored to elderly patients who are unable to get to the office.
Our home visits are comprehensive and convenient. It allows our physicians to learn first hand about your environment and the impact it has on your health.
We bring the best care to you in order to eliminate barriers to accessing essential healthcare services.

Members (Direct Primary Care) may upgrade their memberships to include home visits.
The Home Visit membership offers:
- Discounted physician visits
- Discounted nurse visits
(The frequency of home visits is assessed and determined by the physician. The membership only offers one physician and one nurse visit per month if deemed necessary by the provider).
One-time visits are set at a base price. One-time home visits will be billed at the time the appointment is scheduled. This does not include other costs of services such as blood work, IV therapy, or medication delivery. All Home Visit appointments must be scheduled at least 1 week in advance based on availability.
